Phoenix Health collects the business and contact information you provide through order requests, wholesale applications, certificate requests, and contact forms. This may include your name, business name, business email, phone number, shipping recipient and address, delivery instructions, licensing or resale references, requested products, and related notes.
How the information is used
The information is used to manage your business account and orders, ship your order, respond to questions, locate requested documentation, and communicate about your purchase. Shipping details are included in the order notification sent to Phoenix Health. Your most recent order's contact and shipping details prefill your next checkout for you to review or update.
White labeling
If you request white labeling, we save your branding choice and any logo you upload with your order. Uploaded files are used to prepare your labels. Phoenix Health receives a download link in the order email that expires after 30 days. You can download your logo from the saved order while signed in to your account.
Customer accounts
When you create an account, we store your name, business name, email address, and a protected password hash. Session cookies keep you signed in. We also store account security records and order requests associated with your account. Your password is not included in order-request emails. Contact Phoenix Health to request a correction or deletion of your account information.
Saved order requests
Selected products are temporarily saved in your browser so you can return to an unfinished order. Submitting an order requires a signed-in account. A copy is saved to your account before you continue to the email-delivery step; saving a request does not confirm email delivery, acceptance, or payment.
Form delivery
Website forms use a third-party form-delivery service to route submissions to Phoenix Health by email. Do not submit payment-card information, personal medical information, or other sensitive personal information through these forms.
Payments are completed in Venmo, through Zelle in your banking app, or by wire transfer through your bank. The payment page provides the recipient details for your selected method. Phoenix Health's website does not collect your payment-service login or banking credentials.
